The Guiding an Improved Dementia Experience (GUIDE) Model is a new Medicare-supported initiative created to help families caring for a loved one with dementia remain safe, supported, and empowered at home. Through participating providers, the GUIDE program offers professional care coordination, caregiver education, and respite services—at no additional cost for those who qualify.
UnaTrades Healthcare Solutions proudly partners with GUIDE-approved organizations to help families navigate the emotional and practical challenges of dementia with confidence.
For eligible Medicare beneficiaries, all GUIDE services are fully covered by Medicare. Families pay no out-of-pocket fees for services provided under the GUIDE program.
You or your loved one may qualify for GUIDE services if:
You have traditional Medicare Part A & B (not Medicare Advantage, PACE, or hospice).
You have a clinician-confirmed diagnosis of dementia or early memory loss.
You live at home or in a community setting—not in a nursing home or long-term care facility.
You voluntarily align with a participating GUIDE provider in your area.
Participation may vary depending on availability in Western PA and Eastern OH.

UnaTrades Healthcare Solutions collaborates closely with CMS-approved GUIDE providers to deliver supportive, home-based dementia care personalized to your loved one’s needs. Through this partnership, qualifying families may receive:
Companion & Personal Care Services to support daily routines
Respite Care to give family caregivers reliable relief
Care Coordination to help navigate resources and next steps
Community Connections for additional support and local programs
Our role is to ensure your loved one receives safe, dignified, and consistent care at home while helping families manage the complexities of dementia care.
Our caregivers receive ongoing training to understand the emotional, behavioral, and cognitive needs of individuals living with dementia. This training focuses on:
Supporting independence and daily functioning
Establishing calming, meaningful routines
Communicating with patience and empathy
Recognizing behavior as communication
Enhancing safety and reducing fall or wandering risks
Adjusting care plans as the condition progresses
From the first signs of memory loss to advanced stages of dementia, UnaTrades Healthcare Solutions is here to support both individuals and families. We bring understanding, patience, and personalized care directly into the home—helping your loved one remain safe, connected, and comfortable.
