What Is the GUIDE Model Program?

The Guiding an Improved Dementia Experience (GUIDE) Model is a new Medicare-supported initiative created to help families caring for a loved one with dementia remain safe, supported, and empowered at home. Through participating providers, the GUIDE program offers professional care coordination, caregiver education, and respite services—at no additional cost for those who qualify.

UnaTrades Healthcare Solutions proudly partners with GUIDE-approved organizations to help families navigate the emotional and practical challenges of dementia with confidence.

Is There a Cost?

For eligible Medicare beneficiaries, all GUIDE services are fully covered by Medicare. Families pay no out-of-pocket fees for services provided under the GUIDE program.

Eligibility Requirements

You or your loved one may qualify for GUIDE services if:

You have traditional Medicare Part A & B (not Medicare Advantage, PACE, or hospice).

You have a clinician-confirmed diagnosis of dementia or early memory loss.

You live at home or in a community setting—not in a nursing home or long-term care facility.

You voluntarily align with a participating GUIDE provider in your area.

Participation may vary depending on availability in Western PA and Eastern OH.
